Creatinine
Kidney function is staged on two axes, not one
Most people arrive at these guides holding a single number from a blood test and expecting it to place them somewhere. Nephrology stopped working that way years ago. KDIGO, the international body that writes the chronic kidney disease guideline, classifies CKD by cause, by GFR category and by albuminuria category together, a framework it abbreviates as CGA. Two of those three come from tests, and one of them is a urine test many people never had.
The definition itself has a time component people miss. KDIGO defines CKD as abnormalities of kidney structure or function present for a minimum of three months. One raised creatinine on one morning does not meet that bar.

The mildly decreased and moderately increased labels are both defined relative to young adult level. KDIGO notes that G1 and G2 are not CKD on their own without another marker of kidney damage.
The two axes compound. KDIGO publishes a risk grid crossing GFR category against albuminuria category, running from green for low risk to red for very high, and states that risk rises with higher stages of both. Someone at G3a with A1 sits in a different square from someone at G3a with A3, on the same eGFR.
That grid is why so many guides in this collection deal with urine rather than blood. The albumin-to-creatinine ratio and the protein-to-creatinine ratio are the second axis, and a creatinine result read on its own leaves half the picture out.
The equation behind your eGFR was rewritten in 2021
If you compare an eGFR from 2019 with one from last year, some of the difference is the equation rather than your kidneys. The 2009 CKD-EPI creatinine equation contained a coefficient that adjusted results by race. The 2021 version refits the same mathematical form without it, following a recommendation from the joint task force convened by the National Kidney Foundation and the American Society of Nephrology.
The shift was not cosmetic. Analysis in Clinical Chemistry found the change produces lower values for Black patients and slightly higher values for everyone else, and that the difference can exceed 10 percent at lower creatinine concentrations and younger adult ages. KDIGO adopted the 2021 equation in its 2024 guideline and added a practice point saying race should be avoided in computing eGFR.
The same task force made a second recommendation that gets far less attention. It called for routine use of cystatin C, a second filtration marker, to confirm eGFR in adults at risk of kidney disease, because combining two markers is more accurate than either alone. KDIGO 2024 turned that into a recommendation: estimate the GFR category from creatinine and cystatin C together where cystatin C is available.
What that means if you are reading old results: an eGFR calculated before 2021 is not directly comparable to one calculated after, and the size of the gap depends on which equation your laboratory used and what it recorded about you. A creatinine inside the reference range does not mean healthy kidneys
Creatinine is convenient rather than sensitive. Muscle produces it at a fairly steady rate, kidneys clear it, and a laboratory can measure it cheaply on a sample already being drawn for something else. What it cannot do is warn you early.
Two NIH-hosted texts put the same number on it. Clinical Methods states that blood urea nitrogen and creatinine values remain within the normal range until more than 50 percent of renal function is lost, and adds that in early kidney disease these tests could create a false sense of security. StatPearls says about 50 percent of kidney function must be lost before a rise in serum creatinine can be detected.
Read that against the staging table above. Filtration can drop from around 120 into the 60s, crossing into a range that would be classified as CKD if it persisted, while the creatinine printed on the report still sits between the reference limits. The number has not lied. It was never designed to answer that question.
Then there is everything that moves creatinine without touching kidney function at all. KDIGO lists the non-GFR determinants directly, and they explain a large share of the surprises in this collection.

The scale of the blind spot shows up in national data. The CDC estimates that more than 1 in 10 US adults, about 37 million people, have chronic kidney disease, and that 87 percent of adults with CKD do not know they have it. A test that only moves after half the function has gone is part of why.
