Note the same trap that applies to high results, running the other way. The flag on your report compares you against a laboratory reference population. A small, sedentary person may sit below that bound permanently and be perfectly healthy, while someone whose creatinine has fallen from 1.1 to 0.7 over two years gets no flag at all despite a substantial change in their own baseline. The masking problem, which is the real reason low creatinine matters

Here is the part that most explanations of low creatinine leave out, and it is the part clinicians actually worry about.

Because creatinine depends on production, someone producing very little can have meaningfully reduced kidney filtration while their blood level still reads normal or low. This is the most concrete harm a low creatinine can cause, and it is not obvious.

Most equations that estimate kidney clearance take creatinine as their main input, and a low creatinine produces a high estimated clearance. In someone whose creatinine is low because their kidneys are excellent, that estimate is correct. In a frail older person whose creatinine is low because they have very little muscle, the estimate is flattering.
