Blood pressure: the single biggest thing you control

If kidney prevention had only one instruction, it would be this. Sustained high pressure in the small arteries feeding each filtering unit thickens their walls and narrows them. Some units are starved and scar. Others are exposed to pressures they were never built to handle and fail from the other direction. Because the kidney also regulates blood pressure through fluid balance and hormone signalling, every unit lost pushes pressure up a little further, which is why untreated hypertension and kidney decline accelerate one another.

Targets have shifted over the years and continue to be debated. For most adults without kidney disease, staying reliably below 140/90 is the baseline expectation. The practical consequence is that the first ten years of diabetes are worth far more than the last ten. Tight glucose control early has effects on kidney outcomes that persist for decades afterward, even if control later slips. The reverse is not true; you cannot buy back scarred tissue with good control at year twenty.

Albumin moves before creatinine does

Catching albuminuria while filtration is still normal is the difference between prevention and management. Miss it, and the first evidence you get is a creatinine that has already been climbing quietly for years. Painkillers: the avoidable exposure nobody counts

Ibuprofen, naproxen and diclofenac constrict the small vessel entering each filtering unit. In a well-hydrated person with healthy kidneys and normal blood pressure, that effect is small and reversible, which is why a three-day course for a sprained ankle is not something to lose sleep over. The problem is a different pattern entirely.

Three situations turn a mild effect into a real one. Regular long-term use, meaning most days for months or years, produces cumulative exposure that has been linked to reduced function over time. Use during dehydration or acute illness removes the kidney’s ability to compensate at exactly the moment it needs to. Low concern

Occasional ibuprofen for a headache or an injury, taken with fluids, in someone with no kidney risk factors and normal blood pressure.

Worth changing

Anti-inflammatories most days for chronic back pain or arthritis, taken for years, without anyone monitoring kidney function.

Genuinely risky

Anti-inflammatories taken during a vomiting or diarrheal illness, or alongside a diuretic and a blood pressure drug from the ACE inhibitor or ARB family.

Two practical points. Acetaminophen, called acetaminophen in some countries, does not carry the same kidney risk profile and is often the sensible first choice for everyday pain, though it has its own strict ceiling for liver safety. And topical anti-inflammatory gels deliver far less drug into the bloodstream than tablets, which makes them a reasonable option for localized joint pain.

If an anti-inflammatory has been prescribed for you, do not stop it because of this page. Some conditions genuinely need them and the calculation is different when a rheumatologist has made it. Ask whether your kidney function is being checked, and how often. That is the right question.

Smoking, weight and movement

Smoking is a kidney issue and almost never presented as one. It damages the endothelial lining of small blood vessels throughout the body, and the kidney is essentially an organ made of small blood vessels. Smokers develop albuminuria earlier, lose filtration faster, and progress from early to advanced kidney disease more quickly than non-smokers with otherwise identical risk profiles. Weight, and the hyperfiltration problem

Excess weight harms kidneys through two routes. The obvious one is that it drives hypertension and type 2 diabetes, which then do the damage. The less obvious one is direct: a larger body demands more filtration, so each filtering unit works at higher pressure and higher volume for years. That state, hyperfiltration, looks fine on a blood test — filtration can even measure above normal early on — while it is quietly wearing the tissue out. In some people it eventually produces a specific pattern of scarring associated with obesity.

Weight loss reduces albuminuria measurably, often within months, and improves blood pressure at the same time. The amount needed is smaller than most people assume; the studies that show benefit usually involve losses in the range of five to ten percent of body weight, not a transformation.

Activity

The standard target of around 150 minutes of moderate activity a week is not a kidney-specific recommendation, but it lands on almost every mechanism that matters here: blood pressure, insulin sensitivity, weight, vascular health and inflammation. Resistance training is a useful addition, with one caveat worth knowing about rather than worrying about. Building substantial muscle raises baseline creatinine production, so a heavily trained person can carry a permanently higher reading with entirely normal kidneys. Salt, protein and what diet genuinely does

Diet advice in this area is where reliable information and confident nonsense sit closest together. Here is what stands up.

Salt. Most guidance sits around five to six grams of salt a day, roughly two to two and a half grams of sodium. Most people in Western countries eat well above that, and the great majority of it does not come from the salt cellar. Bread, processed meat, ready meals, sauces, stock cubes, cheese and takeaway food account for the bulk. Reducing salt lowers blood pressure, and in people who already have protein leaking into the urine it reduces that leak independently. It also makes blood pressure medication work better, which is why a nephrologist will ask about salt before adding another tablet.

Overall dietary pattern. Plant-heavy patterns with plenty of vegetables, fruit, legumes, whole grains, nuts and fish — the sort of eating described as Mediterranean or DASH — are associated with slower filtration decline. Whether that is the food itself or everything else that travels with it is not fully separable, but the direction of evidence is consistent and the pattern is good for you regardless. One caution: these diets are high in potassium, which is beneficial for healthy kidneys and can become a problem once function is significantly reduced. Protein. Here the honest answer is that the evidence is thinner than either side of the argument admits. Very high protein intake increases filtration pressure, which is the same hyperfiltration mechanism described above. In people who already have kidney disease, moderating protein is a standard part of management. In people with healthy kidneys, whether a high-protein diet sustained over decades causes damage has not been settled by good long-term evidence, and confident claims in either direction are running ahead of the data. A reasonable position: there is no need for people with normal kidneys to fear ordinary protein intake, and there is also no good reason to push it to extremes for years on end if you carry kidney risk factors.

Ultra-processed food. Beyond salt, heavily processed foods carry phosphate additives that are absorbed far more efficiently than the phosphate naturally present in food. Phosphate handling is one of the kidney’s jobs and one of the first things to become difficult as function falls. This is a stronger argument in people with existing disease than in prevention, but it points the same way as everything else.

Hydration, without the mythology

Dehydration reduces blood flow to the kidney and is one of the most common reasons creatinine rises temporarily. Repeated episodes of significant dehydration, particularly with heat and physical work, are associated with lasting damage. So staying adequately hydrated is genuinely protective.

What does not follow is that more is better. This is the most common misunderstanding in the whole subject. Beyond the point of adequate hydration, extra water does not increase filtration capacity, does not flush anything out of the kidney, and does not slow decline. A randomised trial of coached higher water intake in people with chronic kidney disease did not find slower loss of function compared with usual intake. Drinking three or four liters a day because a wellness article recommended it achieves frequent urination and very little else.

What adequate looks like

For most adults in a temperate climate, somewhere around 1.5 to 2 liters of fluid a day from all sources, adjusted upward in heat, during exercise, and during illness with fever, vomiting or diarrhea. Pale straw-colored urine most of the day is a reasonable rough guide. Thirst works well for the majority of people; the exception is older adults, in whom the thirst signal weakens.

When more genuinely helps

Recurrent kidney stone formers are the real exception. Producing at least two to two and a half liters of urine a day meaningfully reduces recurrence, which usually means drinking noticeably more than average. That is a specific medical indication with evidence behind it, not general advice for everyone.

There is a ceiling worth naming. Drinking far in excess of what the kidney can excrete, particularly during endurance events where sodium is also being lost in sweat, can dilute blood sodium to dangerous levels. Medication vigilance, and what to ask before a scan

Most avoidable kidney injury in adults arrives through the pharmacy or the radiology department rather than through diet. It is episodic risk: nothing for years, then a concentrated window around a new prescription, a procedure or an illness.

Keep an honest list. Most medication lists are wrong. They omit over-the-counter painkillers, supplements, protein powders, creatine, herbal products and anything prescribed by a different service. The omissions are precisely the items most likely to matter here. Write the real list down, including doses, and take it to appointments.

Illness, heat and endurance events: the days that count most

Chronic kidney damage accumulates slowly, but a meaningful share of it arrives in discrete episodes. A bad bout of gastroenteritis, a heatwave, a marathon, a hospital admission. Each one can leave function slightly lower than before, and a run of them across decades produces a stepwise decline rather than a smooth one.

Sick-day guidance

Many people on long-term medication are given a plan for what to do during an illness involving vomiting, diarrhea, fever or markedly reduced fluid intake. The plan usually involves temporarily pausing certain drug classes, then restarting once eating and drinking normally, because those drugs are helpful in ordinary circumstances and unhelpful when the kidney is already short of blood flow. The classes commonly involved include diuretics, blood pressure medicines from the ACE inhibitor and ARB families, anti-inflammatories, metformin and some diabetes drugs.

This is not something to implement yourself. Whether sick-day guidance applies to you, which of your medicines it covers, and when to restart them are decisions that depend on why each drug was prescribed. Some people should absolutely not pause their medication during illness. Ask your GP, pharmacist or specialist whether a sick-day plan applies in your case, and get it in writing before you need it. Heat

Heatwaves cause a spike in acute kidney injury admissions every summer, concentrated in older adults, people on diuretics or blood pressure medication, and those doing physical work outdoors. The mechanism is simple: fluid loss through sweat, reduced intake, and drugs that limit the kidney’s ability to compensate. Practical protection is unglamorous — drink more than usual, stay out of peak afternoon heat, and be more careful than usual about anti-inflammatories during hot spells.

Occupational heat stress deserves specific mention because it has produced entire epidemics. Agricultural workers in parts of Central America and South Asia have developed chronic kidney disease at extraordinary rates, in young men without diabetes or hypertension, apparently driven by years of repeated dehydration and heat strain during manual work. It is the clearest natural experiment we have that repeated dehydration damages kidneys over time.

Endurance events

A transient rise in creatinine after a marathon or long endurance event is common and usually resolves within a couple of days. It reflects a combination of muscle breakdown, dehydration and reduced kidney blood flow. For most healthy people this is not harmful. What turns it into a problem is the combination of anti-inflammatories taken during the event, inadequate fluid replacement, extreme heat, and repeating the pattern often. The two tests that matter, and how often

Kidney screening is not complicated. It is one blood test and one urine test, and the second is the one people skip.

Test one: creatinine and eGFR

A standard blood test, usually part of a routine panel. The lab converts creatinine into an estimated glomerular filtration rate using your age and sex. It tells you how much filtration capacity is currently working. Its weakness is that it is insensitive early: substantial function can be lost before the number leaves the normal range.

Test two: urine albumin-to-creatinine ratio

A single urine sample, ideally the first of the morning. It detects small amounts of albumin leaking through a filter that is beginning to fail. Its strength is exactly where the blood test is weak: it changes years earlier, often while filtration still looks perfect.

Together they answer two different questions — how much filtering capacity remains, and how leaky the filter has become — and neither substitutes for the other. Someone can have a completely normal creatinine and significant albuminuria, and that person has kidney disease that a blood test alone would miss entirely.

A single raised result is not a diagnosis. Vigorous exercise in the previous day or two, a urinary tract infection, menstrual bleeding, fever and very high blood glucose can all push it up temporarily. Standard practice is to confirm with a repeat sample some weeks later before anything is concluded.

How often to test

Diabetes, hypertension or established cardiovascular disease. Annually, both tests, as a minimum. This is standard care in most health systems and is often done automatically as part of a review, though the urine test is missed more often than it should be.

Family history of kidney failure, or previous acute kidney injury. Annual testing is reasonable to discuss with your doctor, even without other risk factors.

Long-term medication that affects kidney function. Interval set by the drug and your prescriber, sometimes every three to six months.

No risk factors at all. Routine screening of the general population is not recommended in most guidelines, because the yield is low and false alarms have costs. If bloods are being taken anyway, creatinine is usually included.

Over 60 with any other risk factor. Worth asking about, since risk factors accumulate quietly with age and the combination changes the calculation.

One more test is worth knowing about. Cystatin C is an alternative blood marker that does not depend on muscle mass, which makes it useful when creatinine is likely to mislead — in people who are very muscular, very frail, amputees, or on a strict vegetarian diet. What a stable decade actually looks like

People tracking their own results usually want to know what normal drift looks like, so that a small move does not cause unnecessary alarm and a real trend is not dismissed.

Filtration declines gradually with age in almost everyone. The usual approximation is somewhere around 0.5 to 1 mL/min/1.73m² per year from around the age of thirty or forty, though the spread between individuals is wide and a substantial minority of healthy older people show almost no decline at all. That figure is an average, not a rule, and it is often quoted with more precision than the underlying data supports.

Layered on top of that is measurement noise. Creatinine varies from day to day within the same person for biological reasons, and laboratory assays add their own imprecision. Myths worth dropping

“Kidney damage is inevitable with age, so there is no point trying.” Filtration does decline with age, but that gradual drift is not the same as chronic kidney disease, and plenty of people reach eighty with excellent function. Most severe kidney disease is driven by diabetes, hypertension and vascular damage, which are modifiable. Aging sets the background rate; risk factors determine whether you stay near it or fall off it.

“Supplements protect your kidneys.” No supplement has been shown to preserve filtration in people with healthy kidneys. Several herbal preparations have been shown to destroy it. The supplement aisle is a net negative in this area, and “kidney support” formulations are marketing rather than medicine.

“Over-the-counter pain relief is harmless.” The word most people attach to non-prescription drugs is “mild”, and for kidneys that is the wrong frame. Habitual anti-inflammatory use is one of the most common avoidable contributors to reduced function, particularly in older adults on other medication.

“Drinking lots of water flushes the kidneys.” There is nothing to flush. The kidney filters continuously and does not accumulate debris that fluid volume can clear. Adequate hydration matters; excess achieves nothing and occasionally causes harm.

“A detox or cleanse resets kidney function.” Your kidneys are the detox system. No juice, tea, fast or protocol improves on them, and some cleansing products contain compounds that damage the very organ they claim to help.

“Protein wrecks your kidneys.” Oversimplified. High protein intake raises filtration pressure, and protein is restricted in established kidney disease. Whether ordinary or moderately high protein intake damages healthy kidneys over decades has not been demonstrated. Confidence in either direction outruns the evidence.

“I would know if my kidneys were struggling.” You would not. Early and moderate kidney disease produces no symptoms whatsoever in most people. Tiredness, swelling and reduced urine output arrive late. This is the single most consequential misconception in the whole subject.

Situationally: the days that count

During gastroenteritis, heatwaves, endurance events and around contrast scans, be deliberate. Fluids, no anti-inflammatories, and a phone call if urine output drops noticeably.

None of that is difficult. What makes it work is doing it before anything is wrong, when there is no symptom prompting you and no result to react to. Prevention in this area is almost entirely a matter of acting during the years when nothing appears to be happening.

Can you actually prevent creatinine?

No, and you would not want to. Creatinine is the normal breakdown product of the creatine your muscles use for energy, produced continuously by everyone with muscle tissue. Around one to two percent of your creatine pool converts to it every day through a reaction nothing can switch off. What you can prevent is creatinine rising, which happens when the kidneys clear less than they used to. Preventing that rise means protecting filtration, and that comes down to blood pressure, glucose control, avoiding certain drug exposures and not smoking.

What is the single most effective thing I can do to protect my kidneys?

Control your blood pressure. It applies to almost everyone, has the strongest evidence behind it, and works whether or not damage has already started.
